Egg Freezing Package fees includes:
i.
Before the treatment cycle:

Doctor’s consultations and ultrasound scans on the day of signing the consents

ii.
Monitoring of the treatment cycle:

Consultations and ultrasound scans during the treatment cycle
(Unlimited number of visits)

Surcharge for follow-up on Sunday or Public Holiday
(Only if recommended by the doctor)

Remarks:
*1)
If the patient has signed the Egg Freezing consent forms and paid the 1st installment, but fails to start the treatment cycle within 90 days of signing the consent forms for whatever reasons, the patient will be deemed to have given up or terminated the Egg Freezing Package (the Package). Any payments made in relation to the Package will not be refunded in part or in total.
*2)
If the patient terminates the treatment cycle on her own accord or cancel the egg collection procedure due to unsatisfactory ovarian response, any payments made in relation to the Package will not be refunded, either in part or in total.
*3)
On the day of the egg collection, if the patient has ovulated before the start of the egg collection procedure, the procedure will be cancelled and the patient does not have to pay the remaining fee of the Package. However, any payments made in relation to the Package will not be refunded, either in part or in total.
*4)
If the egg collection procedure has been carried out, but no egg can be collected after the procedure, the patient does not have to pay the remaining fee of the Package. However, any payments made in relation to the Package will not be refunded, either in part or in total.
*5)
If the egg collection procedure has been completed and egg(s) is/are retrieved, the patient has to pay the 3rd installment of the Package. If the retrieved egg(s) cannot be frozen due to reasons related to the egg(s) e.g. abnormal egg(s) or immature egg(s) etc., any payments made in relation to the Package will not be refunded, either in part or in total.
Terms and Conditions:

Patients cannot designate a doctor or IVF center when using the Egg Freezing Package (the Package). The ovarian stimulation protocol and the type of medications will be decided by the doctor. The Package fees only cover regular medications, examinations, investigations and services related to the treatment cycle; but do not include medications before the treatment cycle or other special medications such as growth hormone, heparin etc. Gratia Medical Centre (GMC) and Hong Kong Assisted Reproduction Center (HKARC) reserve the final right to decide the scope of medications, examinations, investigations and services included in the Package fees.

The Package fees cover the fees of GMC and HKARC only, and do not include the fees if the patient needs to have examination, prescription, injection or treatment in other hospitals, clinics or IVF centres. The 6-month embryo storage fee included in the Package is only applicable to embryos stored at HKARC or any reproductive technology treatment centres licensed by the Council on Human Reproductive Technology in Hong Kong designated by HKARC.

The Egg Freezing cycle has to be started within 90 days of signing the Egg Freezing consents. Otherwise, the Package will be deemed to have been given up or terminated, and any payments made in relation to the Package will not be refunded, either in part or in total. If the patient starts a treatment cycle after the above-said date, any fees related to the treatment cycle, including those related to ovarian stimulation, egg collection, egg freezing and egg storage, etc., will be charged according to the standard charges of GMC and HKARC.
4.
If the patient chooses to terminate the egg storage before the expiry date of the 6-month storage period, any payments made in relation to the Package will not be refunded, either in part or in total. If the patient still wishes to continue to store the eggs after the expiry date of the 6-month storage period, further storage fee will be charged according to the prevailing standard charges of HKARC.
5.
As long as the Package is still available, patients can use the Package multiple times. However, the charges for the Package will be adjusted from time to time. Patients will be required to pay the prevailing price when using the Package.
6.
If the patient has frozen a batch of eggs using the Package, and then undergoes further egg freezing treatment package before the storage expiry date of the eggs from the first cycle, and the new package also includes an egg storage period, the egg storage period of each package cannot be calculated cumulatively. However, the storage expiry date of all eggs will be based on the storage expiry date of the last batch of eggs.
7.
Any package payments that have already been made are non-refundable, either in part or in total.
8.
GMC and HKARC have the discretion to withdraw from the package arrangement at any time without prior notice, and in such a scenario, the standard charges of GMC and HKARC should apply.
9.
Patients shall deliver the Statutory Declaration (if applicable) and the Egg Freezing consents including CONSENT TO FREEZING AND STORAGE OF EGGS (OOCYTES), CONSENT TO DISPOSAL OF STORED OOCYTES and any other relevant consent forms as applicable attached hereto upon acceptance of the Package.
10.
In case of any disputes, the decision of GMC shall be final.
