I. IVF + ET/FET Package fees includes:

i.

Monitoring of the treatment cycle:

  1. Consultations and ultrasound scans during the treatment cycle

    (Unlimited number of visits)

  2. Regular medications related to the treatment cycle

    (Unlimited stimulation drug dosages)

  3. Surcharge for follow-up on Sunday or Public Holiday

    (Only if recommended by the doctor)

  4. Blood hormone tests during the treatment cycle

ii.

Egg collection:

  1. Doctor’s fee for egg collection (Unlimited number of eggs)

  2. IVF Centre fee for egg collection (Unlimited number of eggs)

  3. Anaesthetist’s fee and medication fee for monitored anaesthetic care (MAC)

  4. Surcharge for egg collection on Sunday or Public Holiday

iii.

After the egg collection:

  1. Insemination fee:

    Conventional insemination or ICSI (Unlimited number of eggs)

  2. Embryo culture including Blastocyst culture fee (Unlimited number of embryos)

IIA. IVF + ET/FET Package fees includes ONE Fresh Embryo Transfer (ET):

  1. Fresh ET:

    1. Doctor’s fee for embryo transfer

    2. IVF centre fee for embryo transfer

    3. EmbryoGlue (only if recommended by the doctor)

    4. Assisted hatching

      (only if recommended by the doctor)

    5. Regular medications related to the treatment cycle (Unlimited drug dosages)

    6. Surcharge for embryo transfer on Sunday or Public Holiday

  2. Embryo Freezing and Storage (If applicable):

    1. Embryo freezing fee

    2. (Unlimited number of embryos)

    3. Embryo storage tubes

    4. (Unlimited number of embryos or tubes)

    5. Embryo storage fee for 3 months

  3. After the ET:

    1. Blood hormone tests

    2. Pregnancy test

    3. One follow-up consultation with the doctor within 3 days of the pregnancy test

OR

IIB. IVF + ET/FET Package fees includes ONE Frozen-thawed Embryo Transfer (FET):

  1. Embryo Freezing and Storage:

    1. Embryo freezing fee (Unlimited number of embryos)

    2. Embryo storage tubes

      (Unlimited number of embryos or tubes)

    3. Embryo storage fee for 3 months

  2. Monitoring of ONE FET Treatment Cycle:

    1. Consultations and ultrasound scans during the treatment cycle (Unlimited number of visits)

    2. Regular medications related to the treatment cycle

      (Unlimited drug dosages)

    3. Blood hormone tests during the treatment cycle

    4. Surcharge for follow-up on Sunday or Public Holiday

      (Only if recommended by the doctor)

  3. ONE FET:

    1. Doctor’s fee for embryo transfer

    2. IVF centre fees for embryo thawing and transfer

    3. EmbryoGlue (only if recommended by the doctor)

    4. Assisted hatching (only if recommended by the doctor)

    5. Surcharge for embryo transfer on Sunday or Public Holiday

  4. After the FET:

    1. Blood hormone tests

    2. Pregnancy test

    3. One follow-up consultation with the doctor within 3 days of the pregnancy test

Remarks:

*1) 

If the patient terminates the treatment cycle on her own accord or cancels the egg collection procedure due to unsatisfactory ovarian response, any payments made in relation to IVF + ET/FET Package (One Ovarian Stimulation Egg Retrieval and One Fresh Embryo Transfer or One Frozen-thaw Embryo Transfer) (the Package) will not be refunded, either in part or in total.

*2) 

On the day of the egg collection, if the patient has ovulated before the start of the egg collection procedure, the procedure will be cancelled and the patient does not have to pay the remaining fees of the Package. However, any payments made in relation to the Package will not be refunded, either in part or in total.

*3) 

If the egg collection procedure has been carried out, but no egg can be collected after the procedure, the patient does not have to pay the remaining fees of the Package. However, any payments made in relation to the Package will not be refunded, either in part or in total.

*4) 

If the egg collection procedure is completed and eggs are obtained, the patient has to pay the 2nd installment of the Package. If the retrieved eggs do not meet the criteria for fertilization such as abnormal eggs or immature eggs, or if there are no fertilized eggs, or there are no embryos available for transfer, the patient does not have to pay the 3rd installment of the Package. However, any payments made in relation to the Package will not be refunded, either in part or in total.

*5) 

If there are no embryos survived the thawing so that there are no embryos for transfer, the patient does not have to pay the 3rd installment of the Package. However, any payments made in relation to the Package will not be refunded, either in part or in total.

*6) 

The Package includes one embryo transfer procedure ONLY, either ONE fresh embryo transfer (ET) or ONE frozen-thawed embryo transfer (FET). If the patient has undergone a fresh ET procedure or an FET procedure as part of the Package, any further FET procedures will be charged according to the standard charges of Gratia Medical Centre (GMC) and Hong Kong Assisted Reproduction Centre (HKARC).

*7) 

The FET procedure included in the Package must be performed within 90 days of the Egg Collection. If the patient undergoes an FET procedure after the above-said date, any fees related to the FET treatment cycle will be charged according to the standard charges of GMC and HKARC.

Terms and Conditions:

1. 

Patients cannot designate a doctor or IVF center when using the IVF + ET/FET Package (the Package). The ovarian stimulation protocol and the type of medications will be decided by the doctor. The Package fees only cover regular medications, examinations, investigations and services related to and during the treatment cycle. The Package fees do not include the fees related to consultations, ultrasound scans, screening blood tests, any medications before the treatment cycle, or other special medications such as growth hormone, heparin, aspirin etc. The Package fees do not include the fees for sperm freezing, Pre PGT-A, PGT-SR or Pre PGT-M registration and/or workup, or PGT tests (PGT-A, PGT-SR and PGT-M). Gratia Medical Centre (GMC) and Hong Kong Assisted Reproduction Center (HKARC) reserve the final right to decide the scope of medications, examinations, investigations and services included in the Package fees.

2. 

The Package fees cover the fees of GMC and HKARC only, and do not include the fees if the patient needs to have examination, prescription, injection or treatment in other hospitals, clinics or IVF centres. The 3-month embryo storage fee included in the Package is only applicable to embryos stored at HKARC or any reproductive technology treatment centres licensed by the Council on Human Reproductive Technology in Hong Kong designated by HKARC.

3. 

If the patient chooses to terminate the embryo storage before the expiry date of the 3-month storage period, or if all embryos have been thawed and transferred before the expiry date of the 3-month storage period, any payments made in relation to the Package will not be refunded, either in part or in total. If the patient still has embryos and wishes to continue to store the embryos after the expiry date of the 3-month storage period, further storage fee will be charged according to the prevailing standard charges of HKARC.

4. 

Any package payments that have already been made are non-refundable, either in part or in total.

5. 

The Package fees will be adjusted from time to time. Patients will be required to pay the prevailing price when using the Package.

6. 

GMC and HKARC have the discretion to withdraw from the package arrangement at any time without prior notice, and in such a scenario, the standard charges of GMC and HKARC should apply.

7. 

Patients shall deliver the Statutory Declaration (if applicable) and the IVF consents including CONSENT TO IN-VITRO FERTILIZATION / EMBRYO TRANSFER, CONSENT TO FREEZING AND STORAGE OF EMBRYOS, CONSENT TO DISPOSAL OF STORED EMBRYOS, CONSENT TO TRANSFER OF FROZEN-THAWED EMBRYOS (if applicable) and any other relevant consent forms as applicable attached hereto upon acceptance of the Package.

8. 

In case of any disputes, the decision of GMC shall be final.
